Personal Information We Hold
To provide you with high-quality dental care, we need to hold personal information about you, including:
- Personal details such as name, age, address, telephone numbers, email address, and general medical practitioner.
- Past and current medical and dental condition.
- Radiographs, clinical photographs, and study models.
- Information about provided or proposed treatment (and its cost).
- Notes of relevant conversations or incidents.
- Records of consent to treatment.
- Correspondence with other healthcare professionals, such as referrals to specialists.
Purpose of Holding Data
We keep comprehensive and accurate personal data to ensure safe and appropriate dental care.
Data Processing and Retention
- We retain your dental records while you are a patient and for at least eleven years after you cease to be a patient, or until children reach the age of 25, whichever is longer.
- Personal data is stored in our secure computer system and/or manual filing system, accessible only to authorised staff. Our computer system has secure audit trails and is backed up daily.
Data Disclosure
To provide proper and safe dental care, we may disclose personal information to:
- Your general medical practitioner.
- Hospital or community dental services.
- Other health professionals involved in your care.
- Dental insurance companies and private dental schemes you are a member of.
Disclosure occurs on a “need-to-know” basis and will be limited to necessary information. In limited circumstances or when required by law, personal data may be disclosed to third parties not connected with your dental care. Otherwise, disclosure requires your specific consent, and you will be informed of these requests.
Your Rights
- Access: You have the right to access and receive a copy of the personal data we hold about you. Requests must be made in writing, with a fee of £10 for records and an additional £40 for x-rays. We will provide the requested information within 40 days.
- Correction: You can request correction of incomplete or inaccurate data we hold about you.
- Erasure: You can request the deletion of your personal data when there is no longer a good reason for us to process it.
- Objection: You can object to the processing of your personal data where we rely on a legitimate interest, and it impacts your rights and freedoms.
- Restriction: You can request the suspension of your data processing under certain circumstances.
- Withdrawal of Consent: You can withdraw your consent for data processing at any time.
If you move to another dental practice, we may loan original x-rays and provide copy notes to that practice free of charge upon written request.
If you do not wish for your personal data to be disclosed or used as described, please discuss this with your dentist. You have the right to object, but this may affect our ability to provide dental care.
